Bilingual LPN Care Manager - Tampa
This position is responsible for care coordination and education to individuals living with HIV/AIDS. The Care Manager will assist the client with maintaining adherence to medication regimen and medical care, provide education about health issues which affect/impact HIV, and collaborate with medical providers to ensure continuity of care. This individual must possess a thorough knowledge of and ability to apply the following essential activities: assessment, planning, implementation, coordination, monitoring for all core components of Care Coordination including case management concepts, service delivery, physical and psychological factors, community resources and healthcare care systems.

Minimum Requirements*
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure
- Experience in clinical or related healthcare setting

- Ability to understand and use word processing and basic computer software programs
- Must be able to speak, read, and write proficiently in English. Bilingual required
- Must possess Valid Florida driver’s license, have accessible transportation and proof of insurance
- Must pass necessary fingerprinting and background checks

Medical Educator
Part-time Positions available in Pinellas and Pasco Counties
The Medical Educator works with individuals living with HIV/AIDS to educate on the importance of treatment and medication adherence. The Medical Educator also assists the client in becoming or maintaining adherence by accompanying them to their medical appointments, linking to services, providing 1-on-1 and group support.
Qualified applicants must have the following:
•Ability to speak, read, and write proficiently in English. Bilingual preferred.
•Ability to understand and use word processing and basic computer software programs.
•Knowledge of HIV as it relates to medical adherence and treatment.
•Two years experience within a social service setting or equivalent preferred
•Bachelors Degree in health related field preferred or comparable years of qualifying experience in related healthcare activities.
•Must possess Valid Florida driver’s license, have accessible transportation and proof of insurance.
•Must pass necessary fingerprinting and background checks.

Outreach and Intervention Educator
Hillsborough County
GENERAL DESCRIPTION: Conduct outreach and networking throughout Hillsborough and Pinellas counties in an effort to educate, increase awareness, recruit and enroll individuals eligible into the program. Assist in co-facilitating group level intervention to program participants. Attend provider and community meetings to increase awareness and create referral network among other organizations and groups.
JOB FUNCTIONS:
- Identify and establish a variety of productive outreach venues and sites throughout the assigned county.
- Conduct outreach at events and in community locations and hangouts frequented by the program’s target population (Black/African American women, Hispanic/Latino women, and Racial/Ethic minority re-entry of all genders).
- Attend community meetings and conduct guest speaking presentations to inform and increase awareness of the program within the community.
- Conduct one-on-one and group prevention education on HIV/AIDS, Other STDs, Tuberculosis (TB), Hepatitis, and Substance abuse.
- Conduct needs assessment and intake on eligible individuals, and provides referrals to behavioral health, intensive inpatient/outpatient programs, and supportive services as needed/requested.
- Assist with group facilitation, and serve as primary facilitator in the absence of the group counselor/therapist.
- Prepare room and prepare participants refreshments for the group level intervention as scheduled.
- Maintain contact and follow-up with participants (e.g. phone calls, postcards, home visits) to increase retention.
- Maintain accurate and updated information (e.g. incentives, attendance, conversations, referrals, etc...) in participant files.
- Maintain comprehensive log of all outreach and educational activities.
- Submit accurate time sheets and other reports on time.
- Provide HIV Testing and Counseling as needed.
- Other duties as assigned.
Performance measures:
- Recruit and enroll a minimum of 15 individuals per quarter, for a minimum total of 60 individuals per contract year.
- Ensure that a minimum of 50 participant graduate from the program each contract year.
- Ensure that all participant files are organized, complete, and updated in accordance with DCF licensing, IRB, program, and organization standards.
- Administer appropriate questionnaire evaluations and follow-up to program participants at Baseline (intake), Exit (3-months), Follow-up (6-months), and Post Follow-up (1-year) intervals.
- Disseminate gift card and transportation incentives at appropriate intervals. Ensure that all incentives are tracked, recorded, and accounted for at all times according to organization procedures.
- Ensure that all participant data is complete, thorough, and submitted to the appropriate contact by prescribed deadlines.
- Conduct community outreach and/or attend community events at a minimum of 10 locations per month.
- Ensure that all group supplies/materials are updated and accurate at all times.
KNOWLEDGE, SKILLS AND ABILITIES:
- Ability to understand and use Microsoft Word, Excel, PowerPoint, and basic computer software programs in order to perform job effectively and efficiently.
- Ability to communicate effectively orally and in writing in English with persons from target groups in a culturally appropriate manner.
- Ability to provide positive feedback, empathy, encouragement and guidance to program participants.
- Ability to display productive leadership behavior and encourage a learning environment for staff and participants.
- Ability to communicate effectively with staff, program participants, service providers and community members.
- Ability to recognize and adhere to professional boundaries.
- Ability to inspire and motivate participants to positive outcomes through positive role modeling.
- Knowledge of Substance Abuse, HIV/AIDS, TB, and Hepatitis as they relate to prevention and medical adherence among special populations.
- Ability to work as a team with Metropolitan Charities, Inc. staff, program participants, providers and members of the community.
EDUCATION AND EXPERIENCE:
- Bachelors Degree in a health related field, or comparable years of qualifying educational/work experience.
- Must have at least one year of experience working within a substance abuse, human, or social service setting.
LICENSES, CERTIFICATIONS AND REGISTRATIONS:
- Must possess and maintain valid Florida driver’s license and proof of insurance.
- Must maintain reliable and accessible transportation for travel throughout the Tampa Bay Area.
- Must obtain HIV/AIDS 501 Training certification within 3 months of employment.
